避難長期化で心のケア課題　睡眠不足や認知症悪化も―「困ったら相談を」・能登地震

　能登半島地震では、避難所生活の長期化に伴う被災者の心のケアが課題となっている。睡眠不足や強い余震への恐怖、認知症の症状が悪化したケースのほか、「死にたい」との思いを抱えた人も確認された。日本精神科病院協会の災害派遣精神医療チーム（ＤＰＡＴ）事務局は「適切な機関につなげることもできるので、困ったことがあれば声を掛けてほしい」と話している。
　最大約８００人が避難した石川県珠洲市の飯田小学校。自宅が全半壊した約９０人が避難を続け、うち約４割が６５歳以上の高齢者だ。ラジオ体操や音楽会など心身の不調を防ぐ取り組みが行われているものの、断水で校舎のトイレは十分使用できず、野菜不足による栄養面の偏りが続く。
　県などによると、精神科医らで構成されるＤＰＡＴは約１４０チーム派遣され、避難者の心のケアに当たった。子どもから高齢者まで幅広い年代から「眠れなくなった」などの声が寄せられ、急激な環境変化で精神的に不安定になった結果、精神科病院などに移ったケースもあるという。
　避難所での精神科医療のニーズは、災害発生から１～２週間でピークを迎える。約７０人が避難していた珠洲市のある避難所では１月下旬、「心的外傷後ストレス障害（ＰＴＳＤ）の女児が１人いる。全体的に心のケアのニーズは高まっている」との連絡が市に寄せられた。
　阪神大震災や東日本大震災では、サイレンを聞いて当時を思い出して倒れるなど、数年を経てＰＴＳＤを発症させたケースも報告された。避難所・避難生活学会の水谷嘉浩常任理事は「生きる意欲がなくなることも災害関連死につながる。国や社会全体で『立ち上がろう』と背中を押し、手厚い支援をすることが重要だ」と話した。（2024/02/02-08:16）

Mental health care for those affected by prolonged evacuation following the Noto Peninsula earthquake on Jan. 1 has become a major issue, with people experiencing sleep deprivation, fear of strong aftershocks, worsening dementia, and in some cases, suicidal thoughts.
   The disaster psychiatric assistance team, or DPAT, secretariat of the Japan Psychiatric Hospital Association calls on people to contact it if they have problems, noting that it can connect them to appropriate organizations.
   An elementary school in the city of Suzu, Ishikawa Prefecture, temporarily housed up to around 800 evacuees after the 7.6-magnitude quake. About 90 people, whose homes were destroyed or severely damaged, remain there, and about 40 pct of them are aged 65 or older.
   While efforts are being made to maintain their mental and physical health, such as "radio taiso" exercises and music concerts, evacuees are unable to freely use toilets due to water outages, and nutritional problems continue due to a lack of vegetables.
   According to the Ishikawa prefectural government and others, about 140 DPAT units consisting of psychiatrists and others have been dispatched to provide mental health care to evacuees.
