医療費、自己負担増を＝診療報酬は引き下げ－財務省提言

　財務省は１日の財政制度等審議会（財務相の諮問機関）の分科会で、医療費の自己負担増や診療報酬の引き下げを提言した。高齢化で増え続ける社会保障費を抑制し、医療保険財政を改善させるのが目的。ただ、負担増となる対象者や医療関係者からの反発が予想され、調整は難航しそうだ。
　高齢者が支払う医療費の自己負担額は現在、７０～７４歳が２割で、７５歳の後期高齢者になると１割に軽減されている。財務省は、医療保険制度を支える現役世代と高齢者世代の負担のバランスを見直す必要があるとし、今後新たに７５歳になった人についても２割の自己負担額を維持することを求めた。
　また、すべての世代を対象とし、病院で受診した際の負担額を一律数百円上乗せしたり、病院で処方された医薬品の自己負担額を引き上げたりすることも提案した。
　医療サービスや医薬品の公定価格である診療報酬に関しては、年末に向けて本格化する２０２０年度の予算編成で、２％台半ば以上のマイナス改定とすることを求めた。
　分科会では、文教科学関連予算も議論。財務省は国立大学への運営費交付金のうち、若手研究者の比率などの指標に基づき傾斜配分している枠について、現行の約７００億円を増額する案を示した。（2019/11/01-13:32）

The Japanese Ministry of Finance on Friday proposed raising the shares of out-of-pocket medical expenses and cutting official medical service fees, with the aim of improving the financial condition of the country's health insurance system.
   The proposals, intended to hold down the social security costs that have kept increasing due to the aging of Japan's population, were made at the day's meeting of a subcommittee of the ministry's Fiscal System Council, which advises the finance minister.
   But the proposals are expected to face opposition from medical personnel and people who will be bearing heavier cost burdens.
   Under the current system, the share of out-of-pocket medical expenses stands at 20 pct for people aged 70-74. The ratio is reduced to 10 pct when covered elderly people become 75.
   Seeing the need to review the balance of burdens between the working generation and the elderly, the ministry called for keeping the out-of-pocket share at 20 pct for people who will newly turn 75.
